I hereby give my consent to the University of Gdańsk (8 Jana Bażyńskiego Street, 80-309 Gdańsk) to irrevocable, unlimited in time and territory, permission to disseminate my image recorded on materials containing my image, in connection with the organization of Quantum Summer School “Q-camp”. The consent includes the possibility of using my image as part of any materials directly or indirectly related to the organization of Quantum Summer School “Q-camp”. My image can be used in all existing fields of exploitation where materials containing it will be used. At the same time, I declare that in accordance with the law, I manage my image on my own, without the mediation of other institutions or third parties.
